List executions

Executions, newest first: gateway requests and/or saved Try It Out dry runs (source, default all). Each item has source and execution_id (g:<id> / s:<uuid>, unique across sources: use it as the key). trace_id of a gateway execution is resolved through the lineage event the gateway wrote for the request (null when the request has none, e.g. a pre-flight block that never identified an agent); a dry run's trace_id is its own (it writes no lineage). Gateway-only paging is keyset on id; mixed paging is keyset on (started_at, source, key).
