Add pack template

/api/policy-packs/:pack_id/templates

Add one rule template to a pack — copied from an existing policy's fields, or freshly authored (frontend supplies the same body either way). Only a custom, not-yet-applied pack can be edited; a seeded reference pack is apply-only, and an applied pack's templates are already live — adding to it here would look like it changed the rules it already created.
