Replay past actions

Re-check real past gateway decisions against TODAY's published rules. Reads gateway_requests; builds each request's canonical input; runs the same pipeline (deterministic) and compares with what the gateway decided then. Nothing is executed and no historical row is touched. Limits, stated rather than hidden: tool-call arguments and unmasked values were never stored, the answer of a blocked/held request was the refusal text (so it is not replayed), and the situation is reconstructed from the request record.
