Delete policy
DELETE/api/policies/:policy_id
Delete a policy (meeting ask: duplicates could only be disabled, never removed). Soft delete — the row is hidden and disabled, its manual attachments are dropped, but enforcement history / approvals keep resolving the policy name through their FKs.
